District 8 League Game Reschedule Form

Rescheduling of League Games requires notification and fee payment
a minimum of 14 days
prior to the original scheduled game date or rescheduled
game date (whichever is earlier).
A $30.00 rescheduling fee must be sent
for each game that is rescheduled
after the Level of Play Cutoff Date (as explained to coaches at the coaches meetings).
The check must be made out to “District
8” and received by the District 8 Referee-In-Chief
before a game will be rescheduled.
